In some cases, the patient’s physical condition could deprive her or his presence at healthcare centers, or resorting to these centers has lots of problems for the patients and their families. In this situation, a care team will be sent to the patient’s home to take care of the patient at home to facilitate the provision of services (even on holidays), based on the patient’s or their families request. This team consists of a specialist physician, general practitioner, nurse, physiotherapist, occupational therapist, social worker, psychologist, and spiritual guardian.
Home care services include:
- Periodically visiting by a general practitioner or a specialist to provide palliative medical services to reduce the health-related problems caused by the disease; such as pain, nausea, vomiting, respiratory problems, ulcers, etc.
- Nursing care includes caring for bed sores, doing catheters, percutaneous endoscopic gastrostomy (PEG), etc.
- Consultation to the patient’s family about the nature and optimal management of the disease.
- Supplying medical equipment needed by patients at home.
- Psychologist and spiritual guardian periodically visiting.
- Family care sessions.
- Tele-counselling by a general practitioner.
- Physiotherapy and occupational therapy.
